Diamond Fork Junior High School Counseling Program
Thank you for taking our survey!
Your answers are important to us because they provide a better understanding of how our counseling program can help students achieve development in the areas of: Academic/Learning (AL), Life/Career (LC), Multicultural/Global Citizenship (MG), and Personal/Social (PS).
These codes, AL, LC, MG, and PS, are at the end of some questions and answers. They represent standards for student mindsets and competencies which help counselors prepare lessons for Guidance Curriculum, design effective individual Planning for College and Career Readiness meetings, and address the everyday needs of students through Responsive Services. Specific student mindsets and competencies are available for you to view at https://schools.utah.gov/file/976fc2d5-ed7b-425b-bf24-ee378dd6f641.
This survey is designed to help us better serve you in the future and is not a form for submitting questions or complaints. If you have a comment or concern, please contact our counseling office. There are about 25 questions. You may answer the questions in any order but please complete the survey before leaving your current browser session.
If you have more than one student who attends our school, please pick one student and answer for that student.
